Idiopathic nodular panniculitis is a rare spectrum of skin disorders characterized by single or multiple, tender or painful bumps below the surface of the skin (subcutaneous nodules) that usually lead to inflammation of the subcutaneous layer of fat (panniculitis). These nodules tend to be 1-2 centimeters large and most often affect the legs and feet (lower extremities). In most cases, idiopathic nodular panniculitis is associated with fever, a general feeling of ill health (malaise), muscle pain (myalgia), and/or abdominal pain. These symptoms may subside after a few days or weeks and may recur weeks, months, or years later. The exact cause of idiopathic nodular panniculitis is not known (idiopathic).

  • The nodules are approx 1-2 centimeters large in size. These mostly affect the legs but can also occur anywhere on the body. It is associated with fever, malaise (ill health), myalgia, arthralgia, and abdominal pain.
  • The nodules can leak oily yellow discharge. These symptoms usually go away in some days or weeks but can also be seen after a few weeks, months, and years.
  • The patient with nodular nonsuppurative panniculitis disease may feel nausea, vomiting, weight loss, and hepatomegaly.
  • If the nodule is dislocated, that may involve the abdomen, mesentery of the small or large bowel, lungs, heart, liver, spleen, adrenal glands, kidneys, scrotum, and cranium.
  • Morbidity and mortality are significant. Nodular nonsuppurative panniculitis may also complicate diabetes mellitus.
  • The perfect and specific cause of nodular nonsuppurative panniculitis is yet not known. But the assumption has been given that fat can cause autoimmune or auto-inflammatory granulomatous reactions.

Is there a cure/medications for nodular nonsuppurative panniculitis?

There is no cure for nodular nonsuppurative panniculitis. The patient may get well in a few weeks or months. Few treatments may help in improving the condition but not completely. Treatment is based on detailed patient history and symptoms occurring therein.

  • Oral medications are prescribed that can help in altering the immune system and can also decrease overall inflammation, and can prevent local skin infections.
  • Few medications may also improve the condition of the patient having nodular nonsuppurative panniculitis such as Chloroquine, Thalidomide, Cycloph, Ibuprofen and Indomethacin.
  • If there occur ulcers, then dressings are applied to avoid any sort of infection. If nodular panniculitis becomes severe than systemic corticosteroids i.e., oral prednisone, is prescribed.
  • Some of the treatments that can turn out to be beneficial are Mycophenolate mofetil, Thalidomide and Clofazimine. Antimalarials like hydroxychloroquine, antibiotics, dapsone, and immunosuppressive drugs like azathioprine and ciclosporin.
  • Mild cases of nodular nonsuppurative panniculitis can be treated with pain relief medication like NSAIDS. Medication like cyclosporin A helps in improving the symptom of regression of subcutaneous nodes and also can be beneficial in improving overall health.
  • Using phendimetrazine tartrate derivative antibiotic clofazimine helps in improving the condition at a slow pace.
  • It is not necessary that the above medications may be effective for everyone. It may vary from person to person according to their symptoms and recovery response.
